Depends on the brand but most measure centre (b/b) to centre (seat tube/top tube joint) ('sh), the centre to top is usually included but varies depending on the brand.
More importantly, the (horizontal/"effective") length of the top tube will differ by as much as 1.5 cm (medium Giant; 58.5 cm v's medium Spesh; 60 cm). May not sound like much but its the same as the difference between a 100 mm and 120 mm stem.

Richard would never,ever let me cycle one of his bikes even his hacker "your just to hard on the bike" sure i had to pay him for the last one before he'd let me cycle the bloody thing haha!!!! So Oz it's a spesh I'm after and its 19" I'm just trying to measure mine so centre of BB to top of centre tube got it, thanks.
a bike is the right size for you if you rest your elbow on the Seat collar and try and touch the bottom bracket with a closed fist...if you can rest your knuckles on the bottom bracket shell then you have the right size, its all about ergonomic anatomics..just make sure I'm standing behind you at the moment you bend over, you'll get either a belt of the Mandingo Missle or a size 9 up the arse !! depending on my mood and if you are wearing lycra or baggies !

Hahaahahaha Hmmmmmmmm.Just tried that and doesn't look good as all my bikes are to big!!! I have stumpy arms though!!!
What was said already is true, in the conventional sense, but the best method is to wait for a full moon, get a goat, draw a pentagram around the goat, the sacrifice it to the dark lord with a blunt axe. If you are a true believer then the handle of the axe should be the same length as your down tube, if not then you are obviously a Christian and should really go to mass instead of riding your poorly sized bike around the woods.
